you may have some questions or you might like for Rita to call you to
schedule an appointment.  Use the form below to let us know that you
would like for Rita to contact you to provide information about our
practice and services or to assist you in scheduling an evaluation.   
(Please do not request that we contact an adult other than yourself,
including spouses and adult children.  We are happy to contact adults
who initiate contact with us or parents calling for a minor child).
